چکیده
A technique for multiple deep-inspiration breath-hold (DIBH) volumetric modulated arc therapy (VMAT) for a lung tumor has been proposed with 10 MV flattening-filter-free beams and an image sensor measuring a distance map to thorax surface. Planning CT images were acquired under a DIBH condition and a clinical target volume (CTV) was contoured. This procedure was repeated five times and an internal target volume (ITV) among the multiple DIBHs was created by integrating the five CTVs. A planning target volume (PTV) was defined by adding an isotropic margin of 5 mm to the ITV. Immediately before treatment, a 30-second half-arc cone-beam computer tomography (CBCT) imaging was performed under another DIBH condition, and the couch was repositioned so that tumor may be located inside the PTV contours. An infrared distance measurement device having laser diodes and an image sensor was attached to the couch, and a distance map to the patient thorax surface was recorded as a reference during still another DIBH condition. A half-arc segmented VMAT beams with two beam interrupts were delivered to the patient under multiple DIBHs, where the delivery time of each of the three segmented beams was 30 seconds. During the beam delivery, the distance map was monitored in real time to confirm that the distance to the thorax surface remained unchanged. In-treatment CBCT images suggested that the tumor position at the time of tumor registration was accurately reHow to cite this paper: Nakagawa, K., Nawa, K., Hashimoto, M., Aoki, S., Kaneko, Y., Yamashita, H. and Haga, A. Dosimetric influence of Flattening Filter (FF) and Flattening Filter Free (FFF) 6 and 10 MV photon beams on Volumetric Modulated Arc Therapy (VMAT) planning in case of prostate carcinoma
Background: In the treatment of prostate cancer, radiotherapy is the potential to increase second primary cancers such as bladder and rectal cancers. The reasons for this potential are more monitor units (MUs), therefore a larger total body dose because of leakage radiation, a bigger volume of normal tissue is exposed to lower radiation doses. This study was designed to compare the integral dos...
متن کاملImpact of flattening-filter-free techniques on delivery time for lung stereotactic volumetric modulated arc therapy and image quality of concurrent kilovoltage cone-beam computed tomography: a preliminary phantom study
To the Editor, Volumetric modulated arc therapy (VMAT) serves as a means for stereotactic hypofractionated treatment of lung tumors [1]. The present authors proposed an efficient VMAT sequence by restricting leaf speeds of a multi-leaf collimator per gantry rotation angle to below 1 mm/degree, thereby reducing the dose delivery time down to 210 s for a D95 prescription dose of 50 Gy in four fra...
متن کاملEffects of flattening filter‐free and volumetric‐modulated arc therapy delivery on treatment efficiency
Flattening filter-free (FFF) beams are available on an increasing number of commercial linear accelerators. FFF beams have higher dose rates than flattened beams of equivalent energy which can lead to increased efficiency of treatment delivery, especially in conjunction with increased FFF beam energy and arc-based delivery configurations. The purpose of this study is to quantify and assess the ...
متن کاملVolumetric Modulated Arc Therapy (VMAT) combined with deep inspiration breath hold (DIBH) technique for adjuvant irradiation for cancer of the left breast
Purpose: We describe here the implementation of a very simple approach of combining Volumetric Arc Therapy (VMAT) and the deep inspiration breath hold technique (DIBH) without any costly equipment, in order to reduce the dose at the heart and decrease the cardiotoxicity risk for left sided breast radiotherapy (RT). Methods and Materials: Forty nine patients were treated with the DIBH-VMAT techn...

PURPOSE To evaluate the dosimetric impacts of flattening filter-free (FFF) beams in intensity-modulated radiotherapy (IMRT) and volumetric modulated arc therapy (VMAT) for sinonasal cancer. METHODS For fourteen cases, IMRT and VMAT planning was performed using 6-MV photon beams with both conventional flattened and FFF modes. The four types of plans were compared in terms of target dose homoge...
